Pragmatic Free Trial Meta

Pragmatic Free Trail Meta is an open data platform that facilitates research into pragmatic trials. It collects and distributes cleaned trial data, ratings and evaluations using PRECIS-2. This allows for a variety of meta-epidemiological analyses to examine the effect of treatment across trials of different levels of pragmatism.

Background

Pragmatic studies provide real-world evidence that can be used to make clinical decisions. However, the use of the term "pragmatic" is not consistent and its definition as well as assessment requires clarification. Pragmatic trials are designed to guide clinical practices and policy decisions, not to confirm a physiological hypothesis or clinical hypothesis. A pragmatic trial should try to be as close as possible to real-world clinical practices which include the recruitment of participants, setting, designing, delivery and execution of interventions, determination and analysis outcomes, and primary analysis. This is a major distinction between explanatory trials, as described by Schwartz & Lellouch1 that are designed to test a hypothesis in a more thorough way.

Studies that are truly practical should avoid attempting to blind participants or healthcare professionals, as this may result in distortions in estimates of the effects of treatment. Methods

In a practical trial, the aim is to inform policy or clinical decisions by demonstrating how an intervention would be implemented into routine care. Explanatory trials test hypotheses regarding the cause-effect relationship within idealised conditions. In this way, pragmatic trials could have less internal validity than explanatory studies and be more susceptible to biases in their design, analysis, and conduct. Despite these limitations, pragmatic trials may provide valuable information to decisions in the context of healthcare.

The PRECIS-2 tool evaluates the degree of pragmatism in an RCT by assessing it across 9 domains ranging from 1 (very explicative) to 5 (very pragmatic). Results

They found that 14 of these trials scored pragmatic or highly sensible (i.e. scores of 5 or higher) in any one or more of these domains and that the majority of these were single-center.
